Fruit Street Medicare Diabetes Prevention Program Delivered via Telehealth With Dietitians
Fruit Street is a CDC-recognition diabetes prevention program delivered via telehealth and live group video conferencing with registered dietitians. Starting on August 1st, Fruit Street expects to be approved as a Medicare Diabetes Prevention Program provider which means that our program will be available to your Medicare patients at no-cost with no co-pay.

The program is 12-months long and includes:
● 26 1-hour long classes on Zoom with a registered dietitian and group of other participants
● Topics cover diet, exercise, sleep and stress management
● Participants receive a free wireless scale when they sign up to track weight loss progress.
● Our app also integrates with devices such as Fitbit.
● Participants can take photos of their food and receive feedback from a registered dietitian.
The diabetes prevention program has been proven to help patients over the age of 60 reduce their risk for developing Type 2 diabetes by more than 71% based on research published in the New England Journal of Medicine: Reduction in the Incidence of Type 2 Diabetes with Lifestyle Intervention or Metformin.

Please fill out this form if you would be interested in referring your prediabetic Medicare patients to Fruit Street's diabetes prevention program. The program is available to them at no-cost. If you are interested, we can send brochures to your office and work with you to look at Electronic Health Record data to identify your patients that are prediabetic based an A1C (5.7-6.4) or fasting glucose test (110-125). Medicare has also created two new Improvement Activities as part of Merit-based Incentive Payment System (MIPS) related to glycemic screening services and glycemic referring services.
